| Call the office and let them know you would like to make an appointment. You can download the patient intake forms from this website and fill them out before the appointment or plan to come 30 minutes early and fill them out at the office. A first office call is 2 hours in length and includes a complete history, possible physical exam and recommendations for treatment and a treatment plan. Please bring any medical records, labwork and related information to the visit. Bring any medications and supplements that you are taking. Payment is due at the time of the visit. Currently we do not accept insurance assignment and do not bill for insurance of any kind. If you wish, we will give you a superbill with the appropriate insurance coding at the time of your visit that you can submit to your insurance company. To find out if your insurance company covers naturopathic care, call your Human Resources representative at your place of employment or your insurance company directly and tell them that your providers are state licensed naturopathic physicians. |
